Case in Point is a free 30 page guide for AOD practitioners on how to best record clinical case notes and undertake case formulation. The resource contains a range of practical hints, tips, instructions and examples as well as overviews of common clinical documentation methods such as SOAP, DAP and the 5P’s model.

NB: Queensland public health AOD Services are advised that work is being undertaken to standardise case formulation processes across the state, therefore there is some possibility for amendments associated with the release of updated clinical documentation linked with CIMHA 5.0.
