Pre Care Review Meeting (5:54) 

In line with good practice for meaningful and collaborative discussions during a Care Review, this video demonstrates the primary clinician meeting with Sandra prior to the meeting to: 1) share relevant documents (e.g. care plan, results of outcome measures etc.); 2) check understanding; 3) support a consumer’s input and preferred language in their care review document and; 4) plan matters such as who the consumer would like to attend the meeting.

 

Active Inclusion Care Review Demonstration – Part I - (11:31) 

This video demonstrates some key features that can be included in a consumer’s first Novel Planned Care Review via videoconference. These include: review of progress and any changes since last review;  how outcome measures can be used to assist the process; seeking collateral from stakeholders; and monitoring of domains such as physical health.

 

Active Inclusion Care Review Demonstration – Part II – (6:15)  

Continuing the Care Review meeting demonstration with Sandra, this final section considers the formulation and recent risk assessment as part of the review, and goals and actions that may need to be updated or included for the next period of care.
