MODULE AIM: | Quality care, particularly for chronic disease, requires a multidisciplinary approach to maximise patient outcomes. Completion of this module will enable practitioners to understand what multidisciplinary options might be available for their patients. This module provides participants with the theoretical understanding of the services that are available from different health disciplines for the treatment of chronic conditions. It will also provide health practitioners with the pathways that may facilitate patient access to multidisciplinary care. |
