3.03 | PRESCRIBER AND DISPENSER TRAINING - Interdisciplinary collaboration and referral pathways

Quality care, particularly for chronic disease, requires a multidisciplinary approach to maximise patient outcomes.

Completion of this module will enable practitioners to understand what multidisciplinary options might be available for their patients.

This module provides participants with the theoretical understanding of the services that are available from different health disciplines for the treatment of chronic conditions. It will also provide health practitioners with the pathways that may facilitate patient access to multidisciplinary care.

By the end of this module, learners will be able to: 

• Identify the role that pharmacists play as part of a multidisciplinary team.
• Describe different multidisciplinary treatment options that are available for persistent pain.
• Identify Medicare rebate pathways available for the management of chronic conditions.
• Describe treatment options and referral pathways for mental health conditions.
• Recall referral options and treatment pathways available for a patient with alcohol and other drug concerns.

This module is accredited for:

• 0.5 hours of Educational Activities and 0.5 hours of Reviewing Performance with RACGP;
• 1 hours of CPD with ACRRM; and
• 1 hours of Group 1 CPD (or 1 CPD credits) suitable for inclusion in an individual pharmacist’s CPD plan which can be converted to 1 hours of Group 2 CPD (or 2 CPD credits) upon successful completion of relevant assessment activities with APC.
