Dr Sandra Sunjic

This presentation will discuss a comorbidity project, which through a co-design process generated solutions to improve the collaboration between Drug & Alcohol and Mental Health Services, to improve integrated care for people with comorbid conditions. The implementation of the recommendations from the project and other achievements in the development of the collaboration between the two services, has led to a model that is achieving sustainable outcomes. The achievements and outcomes will be discussed during the presentation, and how these can be translated to other organisations
